Flame Health Pharmacy are looking to recruit a clinical Pharmacist to join our client. We are searching for someone who is highly motivated, displays positivity and who is confident within professional boundaries.

You must be a registered Pharmacist with the GpHC, with significant post-graduate experience to include auditing, development of policies and procedures, excellent clinical knowledge, attention to detail and good time management skills.

It will be your responsibility to work as part of a multidisciplinary team, collaborating with colleagues to solve medication-related issues, enhance patient outcomes and improve access to healthcare.  In this role, the successful candidate will be responsible for providing safe and patient-focused clinical pharmacy services to two hospitals and outpatient clinics.
 

  • Sourcing and ordering drugs to support the evolving clinical modalities
  • Contribute to the promotion and sharing of best practice and quality improvement in medicines safety
  • Provide expertise in clinical medicines management including prescription and medication queries, implement systems for safer prescribing, deal with acute prescription requests, and medicines reconciliation. 
  • Assist in the implementation of procedures to reduce the risks associated with medicines including medicine reconciliation
  • Support the delivery of the audit programme
  • Supervise/manage technicians working in an area of the service.
  • Provide highly specialised clinical pharmacy knowledge and advice on medicines management matters, including cost-effective prescribing and clinical guidelines for disease treatment
  • Assist with the planning and implementing of new services
  • Support the delivery of medicines management staff training programme

Ideal candidates will be independent prescribers who are required to attend CPPE training days and form‌ ‌an‌ ‌integral‌ ‌part‌ ‌of‌ ‌the‌ ‌team.
